Getting “Inside Out and Outside In” the Relational Montage of an Eating Disorder

Seminar Description:

Eating disorders are disorders of self and mutual regulation. They present particular treatment challenges to clinicians in how they engage and how we welcome bodies – their patients and their own --into the therapeutic dialogue. The ‘self’ is derailed and held in the dissociated realm of eating symptoms and body instability with a thwarted sense of agency—a space where dissociation and defensive omnipotence meet—a space where hunger and limits, appetite and choice—desire and wanting are so fraught—they become banished illusions.

Much contemporary analytic thinking focuses on how to allow patients to voice threatening aspects of self and body, often not encoded in words. For those suffering from eating disorders, disavowed longings are experienced as insatiable, powerful, and dangerous, with an excess of energy and self invested in vigilantly measuring ways of living which lack spontaneity, creativity, or play. Symptoms concretize and defend against the danger inherent in any intimate exchange, which requires tolerance for uncertainty and vulnerability, beyond these patients’ reach. Understanding cultural influence, neurobiology, attachment theory, self regulation, affect regulation, self-states, body-states, and the intergenerational transmission of body image issues will be considered as we explore how one works analytically with ED patients beyond symptom alleviation.
